Global Physician Career · USA

Career in the USA —
all steps at a glance.

Residency is the classic route for physicians without a US licence. Those with a specialist qualification may also enter directly as an Attending Physician or via Fellowship programmes. This path always begins with ECFMG.

Key facts about the US path

Verified information — without glossy promises. What is stated here applies as written.

  • ECFMG certification is a prerequisite for the NRMP Match and for starting any accredited residency
  • USMLE Step 1 and Step 2 CK are required for ECFMG certification — the scoring logic changed fundamentally in 2022
  • Step 2 CS was permanently discontinued in 2021 — the replacement pathway has its own requirements and deadlines that must fit within the ECFMG window
  • ERAS (Electronic Residency Application Service) is the application system — separate from the NRMP Match process
  • USCE (US Clinical Experience): programme-specific, not universally required — individual assessment necessary
  • Seven-Year Rule: An early USMLE date sets a window — those who pause within it can find themselves under unexpected time pressure years later
  • OET Medicine: The result must be available within the ECFMG certification window — the exact timing depends on your target Match year

Deadlines

ERAS and NRMP have their own annual deadlines — early planning is essential. Physicians who passed Step 1 early and then paused may find themselves unexpectedly close to the seven-year limit. Raise this openly in your initial consultation.
