Two documents — one difference
Approbation vs. Berufserlaubnis
Both permit medical practice in Germany — under very different conditions.
Approbation
- Permanent, valid nationwide

Berufserlaubnis
- Temporary, geographically and functionally restricted
- Practice under medical supervision
- Interim solution during the Approbation process
- Regulations vary by federal state
- Enables entry into work and income security during the process
Reform Notice · from 1 July 2026
For non-EU/EEA qualifications, the knowledge exam becomes the standard from 1 July 2026 (Bundestag resolution March 2026). Those without a pending application are starting under different conditions — the transition windows are narrow and authority-dependent. The right timing is now critical.
Third-country physicians in practices: Since 2025, physicians with a third-country qualification and valid Berufserlaubnis may also work in outpatient practices and MVZ as assistants — not only in hospitals. This is new and opens additional options for entering the profession.
